This Admission packet is CHAP specific and is designed for all TEXAS Hospices, those already Accredited or seeking Accreditation. This package includes required paperwork that is completed upon admission with the patient and returned to the office and required paperwork that stays in the patients’ home. This allows your Nurses to use on their lap tops while out in the field or alternatively printed in the office for completion by hand, this is a great tool for compliance made easier. To meet CHAP regulation this package includes the before Accreditation and after Accreditation packet. Please use the appropriate package at all times. (see detailed instruction included)
